"Puffing" left atrium in a smoker!

A 42-years-old male smoker presented with progressive dyspnea and palpitation. 12-lead electrocardiogram revealed atrial fibrillation with controlled ventricular rate. Transthoracic two-dimensional (2D) echocardiography revealed severe aortic and mitral stenosis with dense swirling spontaneous echo contrast (SEC) in dilated left atrium. This SEC in left atrium was 'puffed' intermittently into left ventricle in this smoker patient. Patient was treated medically and after stabilization was referred for surgical intervention [Figure 1] adn [Figure 2].{Figure 1}{Figure 2}
